To help you reach your glowing skin goals, we ask prospective clients to begin with a 1:1 consultation with our aesthetic nurse, Lindsay before diving into a regimen. This helps us assess your skin, discuss your concerns, and design a plan of action just for you!
If you provided your email address at booking, these forms were sent electronically. If not, please arrive 15 minutes early for your first appointment to accommodate the check-in process and fill out the intake forms. Or simply save time in the office by filling it out ahead of time and bring it with you.
Come with a freshly washed face and no makeup. A clean face helps speed up our time together so we can see your skin in its natural state to recommend a custom plan.
We’ll want to know anything and everything that goes on your skin, so make a list of your product names and brands — bringing them in works too.
Skincare is often best left to professionals, so although you might be interested in a specific service, we’ll recommend treatments that will work well with your skin.
